20SP-204D-02 COMPLEX STRUCTURE DEMOLITION PLAN - Use in any project with full or partial demolition of structures with continuous spans > 200 feet, girders with horizontal curvature, field assembled suspension spans, movable bridges, cablestayed bridges, structures with cantilevered spans (including pin and hanger structures), post-tensioned concrete structures, continuous span cast-in-place concrete structures, and truss, tied arch, or other non-typical spans.

Use in all rehabilitation and reconstruction and CPM projects utilizing SMA mixtures with a single mixture quantity above 5,000 tons. Use on shoulders paved in a separate operation with inadequate base conditions identified on the plans and more than 5000 tons of HMA for an individual paving course.

20SP-501I-01 ACCEPTANCE OF HOT MIX ASPHALT MIXTURE ON LOCAL AGENCY PROJECTS - Use on all local agency HMA projects except Road Commission for Oakland County, Wayne County DPS and Macomb County Road Department projects, or when a unique FHWA approved special provision on this topic from the local agency is included in the project proposal. The Special Provision for Hot Mix Asphalt Prices for Adjustments must be used with this FUSP.

20SP-501J-02 FINE TEXTURE MILLING AND MICRO TEXTURE MILLING HOT MIX ASPHALT PAVEMENT - Use on all projects with either of the following: 1) Use on one course, non-freeway mill and resurface projects where the integrity of the existing pavement makes it suitable to allow traffic to be maintained on a milled surface for up to 72 hours and where it is desirable to expedite the project schedule and/or increase production paving. OR, 2) Use on Capital Preventive Maintenance (CPM) projects to mill distressed surface seals and construct another surface seal or use to mill HMA surface deficiencies as a standalone treatment.

20SP-501K-02 PAVEMENT RIDE QUALITY (MEAN ROUGHNESS INDEX ACCEPTANCE CRITERIA) - Use on all new concrete and multiple course HMA paving projects greater than 1 mile long. Also use on the following projects of any length: a) cold milling and one course HMA overlays, b) one course HMA overlays and c) diamond grinding projects. Consult with CFS Division before using on urban non-freeway projects. Fill out and include 20TM501(A155) SP for Ride Quality Limits, and include 20RC501(A320) SP for Bump Grinding and include either the Recommended SP for Managing Diamond Grinding Slurry from Ride Quality HMA or Managing Diamond Grinding Slurry From Ride Quality Concrete in the proposal when using this FUSP. Do not use on Local Agency projects except for new concrete or multiple course HMA paving projects on NHS routes. Add 20RC501-A325-02 SP for Ride Quality Incentive for all projects with Class I Ride Quality and/or Class II Ride Quality with Incentives.
